Important note: This application is a SF -1187 is a government form. Since this is a government form, and not considered an internal union form, it can be placed in a blue envelope and mailed to the Union President. You are allowed to use your office copy machine to make a copy for yourself.

SF 1188. Is the form to withdrawal from the Union. (Now why would you want to do that?) when you have so many reasons to join. For more information about stopping dues, refer to Article 42 of the contract.
